Dr. Kerri personally treats every child every visit, including routine cleanings and checkups.

Please schedule an appointment at a convenient time for you and your child, and be sure to arrive promptly.
the responsible parent and/or guardian should bring insurance cards and their driver's lisence.

Please bring, if available, current x-rays and/or written referrals.
If you are coming from a referring dentist
Confirmation policy
This office will contact you at least 48 hours prior to your scheduled appointment to request confirmation. If a message is left for you, it is important you return the call. All appointments must be confirmed with this office at least 48 hours prior to the appointment. If your appointment is not confirmed it may be cancled and provided to a patient on short call with this office. Patient deposits may be forfeited.

You must be on time for your appointment, there is no grace period. Failure to provide 48 hour notice of cancellation may result in dismissal from this office.

Should you fail to arrive for an appointment without notice of cancellation at least 48 hours in advance, you may be subject to additional charges such as a $30 registration fee for future appointments. You may be added to the office short call list or dismissed from this practice.
